The student will observe and uphold the policies, rules and regulations of the school, the business and all other professional environments.

In the event of a necessary absence, advance notification will be provided to both the instructor/educational representative and the employer/business mentor.
The student will complete all required employment forms, records of experience, evaluations, and other assignments as outlined in this agreement.
In consideration of being permitted in an internship with the company listed herein, myself, heirs, administrators forever discharge Sinclair Community College, the employees thereof, and the State of Ohio, from any and all actions, causes of action, claims, charges, demands, complaints, damages, injuries, costs, loss of services, expenses and compensation on account of or arising while in, on, en route, to or from said internship.

I authorize the Office of Work-based Learning (OWork-Based Learning) to release my contact information and resume to employers on my behalf in order to assist me in my job search. I agree to follow up with prospective employers in a timely manner and conduct myself in a professional manner. Upon accepting an internship, I agree to complete it as agreed upon with an employer and notify my Work-Based Learning coordinator and instructor if any changes in employment occur. I understand that this is an educational program and internships are intended to be entry-level assignments; they are considered temporary and do not qualify for benefits. Although the internship may be a requirement for graduation, I realize that Sinclair Community College, my instructor, and the OWork-Based Learning are under no obligation to find an internship for me and that securing an opportunity is ultimately my responsibility. I understand that the OWork-Based Learning reserves the right to discontinue services when deemed appropriate.
