Our goals through this process are to:
Take your time: The following six, multiple choice questions may take up to 30 minutes to complete. Consider each question thoughtfully. There are no "right" answers—we value honest reflection about where your organization currently stands.
Select best matches: For each section, choose the option that most closely reflects your organization's current reality, not aspirations for the future.
Be authentic: We recognize organizations are at different points in their equity journeys. Your honest responses help us better understand how to support your work.
We recommend discussing these questions with your leadership team to ensure responses accurately reflect your organization's current practices and values. The person who is most knowledgeable about your organization should complete this form. The email you enter into this form should be the primary contact for this submission.
After you complete the form, our team will carefully review your responses to gain a deeper understanding of your organization's values and approaches. We will email you approximately two weeks from submission with specific information about the next steps in our funding process.
